It's Review Time!

Employee Self-evaluation Form

All employees are expected to complete the review form and return it to their manager no later than end of day Friday, 2/13.

Please note:

• You must complete the form electronically and submit a saved Word document via email or approved submission method.
• Printed copies for initial submission to managers are not acceptable.
• If you do not understand something on the form, experience technical issues completing or submitting it, or have any other related issues, please reach out as soon as possible.

Managers will be scheduling sit-down performance review meetings during the first and second weeks of March.

This form is intended to help guide the performance review discussion. These reviews are not the only factor that will be factored into compensation calculations, but it is an important input that will be considered and should be taken seriously.
